As a Care Coordinator, you will coordinate and manage care services for clients in the Hampstead and Camden area. You will also conduct client assessments and create personalized care plans and match clients with suitable caregivers based on their needs and preferences. Please note, this position would suit a compassionate individual with a passion for helping others.
In your first few weeks in this Care Coordinator role, you can expect to:

Monitor and review care plans to meet changing needs

Excellent organisational and time-management, communication and interpersonal skills
Understanding of person-centred care principles
Knowledge of the local area and available resources
Familiarity with CQC regulations (desirable)
NVQ Level 3 in Health and Social Care (preferred)
