SOCIAL WORKER – TRANSITIONS & PREPARATION FOR ADULTHOOD SERVICE

Location : Birmingham

Our client is looking to increase their current team of Social Workers in their Transition & Preparation Department. You would be responsible for providing support and guidance to young people aged 18-25 who are preparing to transition to adulthood. They may have learning and/or physical disabilities.

Duties and Responsibilities :

• Developing and implementing care plans that meet the physical, emotional and social needs of young people.
• Supporting young people in accessing appropriate education, training, and employment opportunities.
• Advocating for the rights of young people and ensuring that their needs are met.
• Developing and maintaining positive working relationships with families, carers, and other professionals.
• Providing advice, guidance and support on a range of issues including health, education, housing and finance.
• Ensuring that young people are aware of their rights and that their opinions and views are taken into consideration.
• Maintaining accurate up to date records of young people and their care plans.
• Participating in training and development programs to build knowledge and skills.

Requirements :

• A degree in Social Work from an accredited institution
• 2 Years plus experience in a Social Worker position
• Strong organisational, communication, and time management skills
• The ability to work both independently and as part of a team
• Good problem solving skills
• A commitment to professional development
• Enhanced DBS with Adult Barring
• Full driving licence
