Our client is looking to increase their current team of Social Workers in their Transition & Preparation Department. You would be responsible for providing support and guidance to young people aged 18-25 who are preparing to transition to adulthood. They may have learning and/or physical disabilities.
Duties and Responsibilities :
Developing and implementing care plans that meet the physical, emotional and social needs of young people.
Supporting young people in accessing appropriate education, training, and employment opportunities.
Advocating for the rights of young people and ensuring that their needs are met.
Developing and maintaining positive working relationships with families, carers, and other professionals.
Providing advice, guidance and support on a range of issues including health, education, housing and finance.
Ensuring that young people are aware of their rights and that their opinions and views are taken into consideration.
Maintaining accurate up to date records of young people and their care plans.
Participating in training and development programs to build knowledge and skills.
Requirements :
A degree in Social Work from an accredited institution
2 Years plus experience in a Social Worker position
Strong organisational, communication, and time management skills
The ability to work both independently and as part of a team
Good problem solving skills
A commitment to professional development
Enhanced DBS with Adult Barring
Full driving licence
