Team Leader, Senior PractitionerChildren and Young People Inpatient and Hospital Settings Hours 37 Contract type Permanent (Fixed term funded post, current funding to March 2024) ESSENTIAL – DRIVER – You will be required to work across multiple sites and clients therefore Driving is essential to allow you to manage your caseload of appointments.Are you an experienced Senior Practitioner who has with children and young people or within inpatient and hospital settings and are looking for a dynamic role that makes a positive impact on the lives of children and young people with learning disability, autism or both? We are looking to recruit 2 Learning Disability and Autism Leads who will have a dedicated area of work to develop lead and deliver nonclinical interventions and practice within the existing Service. The roles will focus on personalised care and support planning for children and young people who are autistic, or have a learning disability or both and their families alongside other health, social care, education and other professionals in the following areas:

• Tier 4 children and young people inpatient and hospital settings

These can be particularly challenging situations and circumstances for young people and families to navigate and as the Learning Disability and Autism Lead, you will use the knowledge, experience and skills you have relating to your specialist area, alongside other professionals and organisations to ensure that the young person remains at the centre of the care, support and treatment they are in receipt of. You will also work with the wider team by sharing learning, experience, skills and knowledge to support other young people within the community who may be presenting with higher levels of need. As Learning Disability and Autism Lead in children and young people inpatient and hospital settings you would be expected to:

• Have responsibility for a caseload working with children, young people, their parents/carers and families to navigate what can be a complex health and social care system so that their needs are met, and they remain in an inpatient or hospital setting for the required shortest of time.
• Have and build upon your working knowledge of how children and young people admitted into inpatient and hospital settings operate and how statutory and community-based support services operate and support these children and young people across West Yorkshire.
• Understand the mental health and wellbeing needs of autistic children and young people and those with a learning disability or both in an inpatient or hospital setting including the importance of the sensory environment and the impact of trauma and adversity.
• Understand situations and key indicators of risk and vulnerabilities that can result in inpatient admissions under the Mental Health Act.
• Hold working knowledge of community support available – focusing on supporting discharge into the community and family and preventing inpatient admissions.
• Understand the barriers that exist within services for autistic children and young people and those with a learning disability
• Build strong collaborative links.
• Understand and be responsive to challenges within existing systems and highlight these through the development of work.
• Be the point of contact for the wider team of professionals who may be supporting children and young people who are at risk of hospital admission.
• Share skills and knowledge with others within the service regularly in team meetings, informal training and learning sessions for professionals, parents and wider communities.
